Care Partner / Case Manager
Visa sponsorship
Community Services · Sydney, NSW, Australia · Full-Time · $80,000 - $95,000/yr · Visa: Employer sponsored
For an aged care professional aiming to move from hands-on support into case management, this Sydney role offers a genuine step up in responsibility. An aged care provider delivering person-centred care to older Australians in their homes, under the Support at Home program, is looking for an experienced and compassionate Care Partner / Case Manager.
The position builds skills that carry weight across the sector: leading care coordination, managing individual support plans and supervising Support Workers. You would operate autonomously in a flat structure, accountable for making sure clients receive appropriate services when they need them. Visa sponsorship is also available for the right candidates.
